As a trusted provider for the Consolidated Waiver through the Department of Human Services in Pennsylvania, we empower individuals with intellectual disabilities and developmental disabilities to thrive in a safe, supportive group home setting.
Each person admitted to our program will work with our team to develop an Individual Service Plan tailored to their specific needs and preferences. All available services are designed to promote independence, community living, and holistic well-being for those in our care.
Our Residential Services include a wide range of supports, such as:
- Medication Management: Our caregivers are trained to assist residents, as needed, with taking their medication including reminders for timing and dosage, tracking multiple medications, and following prescription instructions.
- Nursing Services: We offer more specialized nursing care as needed to accommodate each resident’s unique medical situation.
- Behavioral Support: Our caregivers follow trauma-informed, person-centered practices for effectively assisting residents in managing behavior in a way that respects their autonomy and adjusts to their personal triggers and support needs.
- Personal Care Assistance: we assist with daily living activities such as dressing, self-feeding, oral hygiene, hair care, grooming, toileting, and bathing in accordance with each resident’s needs.
- Community Engagement: Our caregivers connect residents with the resources, programs, and activities in their local community to foster connection with others and promote a sense of inclusion and fulfillment.
- Recreational Programs: Each resident’s interests, goals, and abilities inform a custom-tailored recreational calendar to support their choices, offer opportunities for self-expression, and foster connection with their passions and community.
- Nutritious Meals & Snacks: We are committed to providing quality, nutritious, and readily available meals and snacks to support the holistic well-being of each resident.
- Housekeeping & Laundry: Our caregivers assist in cleaning and laundry to aid each resident in the tasks of everyday living and help them maintain a high quality of life.
- Transition Assistance: We offer, as needed, the assistance of a social worker to help smooth the transition between a person’s previous living environment and our residential program, to ensure their continuing needs are met, and to provide the resident with the opportunity to participate in planning their future care.
We currently offer services to adults in Indiana and Armstrong counties in Pennsylvania.
